Every pet’s records in one place, ready for the next vet visit. A 34-page printable binder for up to six dogs and cats, with a fillable version. Instant download.
Description
The vet asks when the last vaccine was. You know it was spring. Or maybe last autumn. The record is in a drawer, a photo is somewhere on your phone, and the medication dose is on a sticky note on the fridge. With two or three pets, the details blur together fast.
This printable pet health binder puts every pet’s records in one place. Vaccines, medications, weight, vet visits, symptoms and emergency contacts each get their own page. Every pet gets their own colour. Print it, fill it in once, and the next vet visit starts with answers instead of guesses.
Purpose: a printable pet health binder for multi-pet homes
Pet records tend to scatter. Each vet gives you a different printout, vaccine cards get lost, and medication changes live in your memory. That works until an emergency, a new vet or a pet sitter needs the information quickly.
The binder gives every record one home. Each pet has a colour on the Pet Index, and matching colour dots appear on every record page. Flip to the right colour and the right page, and the answer is there.
Why paper still helps
Apps come and go, phones run out of battery, and a sitter may not have access to your accounts. A binder on the shelf works for anyone in the house. The fillable version gives you a digital copy too, so you can keep both.
Materials & make: what is inside the binder
The binder has 34 pages in four sections, and each page has one clear job.
- Six colour-coded pet slots, with a matching colour dot on every record page
- A one-page Vet Visit Summary to fill in before appointments and hand over at check-in
- A medication tracker with room for six medicines, morning and evening
- Vaccination record, weight tracker, symptom tracker and medical history
- A monthly home health check and a yearly care planner
- Emergency Info Sheet and Pet Sitter Guide
- Wallet emergency cards that print at true bank card size (3.5 × 2 in)
- A lost pet flyer with tear-off phone tabs

The files you get
Seven PDF files arrive in one zip download. Colour versions come in US Letter, A4 and A5. Ink-friendly versions come in US Letter and A4 for home printers. Fillable versions in US Letter and A4 let you type on screen.
Fit: who the pet health binder is for
- Homes with more than one dog or cat
- Pets on regular medication or with ongoing health needs

How to set it up in one evening
- Download and unzip the folder.
- Choose colour, ink-friendly or fillable, and your paper size.
- Print at 100% or “Actual size”. Use cardstock for the wallet cards.
- Fill in the Pet Index with each pet’s name and colour.
- Add vaccine dates, medications and vet contacts from your existing papers.
- Keep the binder somewhere a sitter can find it.

Using it with a pet sitter
Before you travel, fill in the Pet Sitter Guide and the Emergency Info Sheet, and leave the binder on the kitchen counter. Your sitter finds feeding times, medication doses and your vet’s number in one place. Print the wallet emergency card for yourself, so the vet and your emergency contact are in your purse if something happens while you are away.
Going away for more than a night? The Pet Sitter & Emergency Pack adds a full sitter handbook with routines, house notes and a vet form. For cards in every wallet, on your keys and on the front door, see the Pet Emergency Card Kit.
The day it is for
Your dog needs an unplanned vet visit on a Saturday morning, at a clinic you have never used. You take the binder. At check-in you hand over the Vet Visit Summary, with vaccines and medications on one page. The vet reads it in a minute, and the appointment is about your dog, not about remembering dates.
For older pets, the Senior Pet Care Pack adds daily and weekly logs. More printable tools are on our tools page.
Care
Keep the printed pages in a ring binder or folder, out of direct sun. Reprint any page when it fills up; the files are yours to print for your own household as often as you need. Update the medication tracker every time a dose changes.
This is a record-keeping tool, not veterinary advice. Follow your vet’s guidance on your pet’s care.
